  • Patient Reactions, Nursing Actions, Restart/Stop Times: Document any patient reactions, nursing actions taken, and restart or stop times during the infusion.

  • Only one initial service may be coded per encounter. 

  • A bolus of prepackaged fluids or specific medications should be coded as therapeutic. 

  • Start and stop times determine how to calculate the hours. 

  • The additional hour can be included only when the infusion has lasted more than 30 minutes into the second hour. 

  • The fluid used to administer drugs is considered incidental hydration and is not separately payable.

  • 2nd hour: Infusion duration between 91 and 150 minutes is considered one unit of additional subsequent service code. 

  • 3rd hour: Infusion duration between 151 and 210 minutes is considered two units of additional subsequent service code.

  • Secondary Infusion Services - it is important to keep in mind that the secondary infusion services, such as sequential infusion and concurrent infusion, have their specific coding requirements.   

  • Infusion and E&M Services are also important -When an infusion service is accompanied by a significant separately identifiable evaluation and management (E&M) service, it's essential to report the appropriate E&M CPT code along with modifier 25. This modifier indicates that a separate and significant E&M service was provided. It is not necessary to have a different diagnosis for the E&M service provided on the same day as the infusion.By utilizing the appropriate E&M codes and modifiers, providers can accurately bill for both infusion services and associated E&M services.
